Understanding Creatine Kinase

Creatine Kinase (CK) measurements are crucial for diagnosing and treating myocardial infarction and muscle diseases, like progressive Duchenne-type muscular dystrophy.

CK, primarily found in skeletal muscle, cardiac muscle, and brain tissue, shows elevated levels in various conditions, including myocardial infarction and muscle disorders. In myocardial infarction, CK levels peak 24 to 36 hours after chest pain onset, sometimes exceeding ten times normal levels. Notably, Reye’s Syndrome can cause a remarkable up to 70-fold increase in CK activity due to severe encephalopathy.


This CK procedure, a modification of the IFCC method, involves catalyzing a phosphate group transfer from creatine phosphate to adenosine diphosphate (ADP), yielding creatine and adenosine triphosphate (ATP). The ATP further contributes to glucose-6-phosphate and ADP production from glucose, catalyzed by hexokinase (HK), which requires magnesium ions. Glucose-6-phosphate oxidation by glucose-6-phosphate dehydrogenase (G6P-DH) forms NADPH and 6-phosphogluconate. The increase in absorbance at 340/660 nm, due to NADPH formation, directly correlates with CK activity.
