Understanding Creatinine Kinase

Measurements of creatinine are pivotal in diagnosing and treating renal disease. Moreover, serum creatinine measurements are beneficial for evaluating kidney glomerular function and monitoring renal dialysis. However, serum creatinine levels may not promptly reflect early renal damage compared to blood urea nitrogen (BUN), especially during hemodialysis treatment for renal failure. Both serum creatinine and BUN are instrumental in distinguishing prerenal and postrenal (obstructive) azotemia. In particular, an increase in serum BUN without a concurrent rise in serum creatinine is indicative of prerenal azotemia, whereas with postrenal azotemia, both BUN and creatinine levels rise, with BUN showing a disproportionately greater increase.

Serum creatinine levels vary with factors such as age, body weight, and sex. They may be lower in individuals with relatively small muscle mass, cachetic patients, amputees, and older persons. Hence, a serum creatinine level within the usual range does not necessarily rule out impaired renal function.

Methodology

The Creatinine procedure is a kinetic modification of the Jaffe procedure, where creatinine reacts with picric acid at alkaline pH to form a yellow-orange complex. However, this reaction is not entirely specific to creatinine, as other reducing substances like glucose, pyruvate, ascorbic acid, and acetoacetates can also react with picrate to produce a similar color. However, Fabiny and Ertingshausen observed that alkaline creatinine picrate reaches maximum color development at a different rate than pseudo-creatinine material. Cook further leveraged different reaction rates of alkaline picrate-positive substances to enhance specificity with the Jaffe reaction.

The rate of change in absorbance at 520/800nm is directly proportional to the creatinine concentration in the sample.
